WHEREAS THE UNDERSIGNED wishes to participate in certain acts, practices, procedures, and games commonly known as “BDSM” (hereinafter referred to as the “Activity”) under the facilitation of TRC CONSULTING. THE UNDERSIGNED WARRANTS that they have no physical limitations, medical ailments, or physical or mental disability that may limit, impair, degrade, or otherwise inhibit or prevent participation in the Activity other than those disclosed to TRC CONSULTING. IN CONSIDERATION OF permission to participate in the Activity and other good and valuable consideration, the sufficiency of which is hereby acknowledged, the undersigned, their agents, heirs, successors, insurers, and assigns (hereinafter referred to as the “RELEASORS”) hereby release and discharge TRC CONSULTING, its directors, officers, shareholders, subsidiaries, parents, employees, agents, subcontractors, or assigns (hereinafter referred to as the “RELEASEES”) from any and all claims, inclusive of any claim of negligence, breach of duty, breach of warranty, or breach of contract that may be alleged to arise in connection with the Activity. THE RELEASORS ACKNOWLEDGE that participation in the Activity carries inherent risks and hazards that cannot be eliminated with the exercise of reasonable care and due caution. THE RELEASORS AGREE not to commence or continue any action or proceeding or make any claim against any person or entity who may be entitled to claim contribution and indemnity from the Releasees. The Releasors further agree to fully indemnify the Releasees in respect of any awards, costs, damages, disbursements, and legal fees incurred as a result of any such claim for contribution and indemnity made against the Releasees. THE RELEASORS ACKNOWLEDGE that execution of this Waiver and Indemnity Agreement will result in the extinguishment of certain of their legal rights, including their right to commence or continue legal action directly or indirectly against TRC CONSULTING. in connection with the Activity. THE RELEASORS FURTHER ACKNOWLEDGE that they have read this Waiver and Indemnity Agreement in full. THE RELEASORS AGREE that this Waiver and Indemnity Agreement shall be construed in accordance with and governed by the laws of the Province of Ontario.

I acknowledge that HM Shahrazad reserves the right to book only those inquiries they feel comfortable booking and is under no obligation to take you on as a client unless they feel an enthusiastic “yes” about participating in the experience with you. I acknowledge that HM Shahrazad DOES NOT offer fulfillment of complex wardrobe requests, heavily scripted scenes, sexual services, high heel shoe wearing, complex invasive medical play (other than needles), suspension bondage, scat play/spitting/golden showers/facesitting/WAM scenes. I acknowledge that HM Shahrazad is no longer offering these services full-time and that I may not get my first choice of date/time, but that every effort will be made to find a mutually agreeable slot. To that end, “Urgent” or “Last Minute” requests will not be accommodated. I acknowledge that HM Shahrazad may take up to 4 business days to respond to this inquiry.
